Tuesday, October 05, 2010 (EST) - Alzheimer's Disease/Dementia Caregivers Support

An ongoing monthly support group for all people who have a loved one impacted by Alzheimer's Disease or Dementia. The group meets the first Tuesday of every month beginning March 4, 2008 at the Groton Public Library.



For all spouses, children, family members, community  and facility caregivers of those with Alzheimer's Disease and Dementia.  The group will offer support and encouragement, teach caregiving skills, provide information and resource literature and teach self care skills while caring for others.  Registration is requested.  Contact Deb Kozisek at (860)536-5601 ext. 24.

    Responsible for establishing, updating, and communicating the visit schedule of all interdisciplinary staff members.

    Provide Palliative Care Services for our subsidiary Beacon Palliative Care Services, Inc.

  • RN Case Manager
    The RN Case Manager provides nursing care and hospice case management to hospice patients and families. Implements nursing interventions and coordinates the Patient/Family Interdisciplinary Plan of Care.
